How to Book

Choose type (Initial Consultation, Follow-Up, or Welcome to Wellness.). Then click on next.
2. Date & Time
Choose a date and time that is convenient for you and click on next.
3. Fill Out Form
Fill out the appointment form with your name, address, email, phone number, and password.
4. Confirm
Confirm your appointment and Receive confirmation email
If the days and times don’t fit your needs, please use our contact form.
Cancelation & Rescheduling Policy
We value your time and ours. If you need to reschedule or cancel, please provide at least 24 hours’ notice. Missed appointments or late cancellations may be subject to a fee, as outlined in our Fees & Insurance page.
